This ANNUAL APPLICATION AND AGREEMENT is made and entered into by and between Harrison Hills Golf and Country Club, hereinafter called the club and the above named applicant(s) , including spouse if applicable, hereinafter jointly called and severally called the applicant(s) or the member(s) , and witnesses that in consideration of the promises on the part of the club and the applicant(s)/member(s) and they both agree as follows:
- 1. This annual membership is for the current calendar year 2007. This membership must be renewed on an annual basis.
- 2. The applicant shall pay the total sum of annual dues via one of the following three ways: (please check applicable selection)

- 3. Upon submission and acceptance of this application, the applicant shall immediately become a member of the club and shall be entitled to the full rights and privileges of membership and be subject to the rules and regulations of the club.
- 4. This application/agreement is a binding contract and the parties agree to be bound by the terms hereof.
- 5. The member releases the club of all responsibility and liability and the member accepts all responsibility and liability for payment of damages to property and/or persons caused by or to the member.
